You must have a digital copies/photos of your completed and signed Sports Physical form and health insurance card to upload to this online registration form. You will not be able to complete this form without uploading these items.

Please note this is a 2 Step Registration. After submitting this form, the confirmation message will have a link to 8to18.com for Step 2. If you do not have an 8 to18 account already, you will need to create one. Both steps of the registration process must be completed in one sitting.

Once this Step 1 form has been completed, you will receive an email confirmation. If you do not receive an email confirmation, that means you did not complete this form in it’s entirety.
